Julia Lockyer, PsyD is offering evaluations for ADHD and learning disabilities.  These evaluations include a meeting with the client (and parents, if client is a minor), the testing, the write-up of the results, and a discussion of the results.  The write-up of these evaluations can be used for Individualized Education Programs (IEPs), 504 plans, or other disability considerations.

The cost for the evaluations are as follows:

• ADHD evaluation and assessment for child ages 6-12: $1450
• ADHD evaluation and assessment for individual ages 13+: $1200
• ADHD and learning disability evaluation and assessment for child ages 6-12: $2250
• ADHD and learning disability evaluation and assessment for individual ages 13+: $2000

We are currently only able to accept Tricare insurance for these evaluations.  For individuals with Tricare Prime, a referral from a primary care manager is mandatory.  For individuals with Tricare Select, no referral is needed.
